Admission of Ex-Offenders

Applicants who are felony ex-offenders must complete the Supplemental Admissions Information form for Ex-Offender. Ex-offenders must be paroled at least six months after his or her last incarceration. No applicant will be process until this supplemental application is completed and returned.
